Not another tier list. I took 89,316 public forum posts, turned every one into a structured claim â what was done, what it supposedly did, over what timeframe, and how good the evidence was â and counted 32 interventions. The question this answers isn't "what's popular", it's what do people report working, and how many of them agree.
The best-evidenced things are the ones with a clinician in the loop (jaw surgery 71.4% at tier 3+, braces 51.6%), the cheapest free thing that works is sleep (34.3%), and the two most-stanned free things â mewing (26.6%) and bonesmashing (20.0%) â sit at the bottom with the most enthusiasm and the least evidence.
Share of claims reaching tier 3+ (consistent human reports), among interventions with at least 40 claims so one thread can't move the number.
| intervention | claims | tier 3+ | cost | risk |
|---|---|---|---|---|
| Jaw surgery (orthognathic, bimax) | 262 | 71.4% | £0 NHS / £8k-30k | high |
| Braces, aligners & palate expansion | 335 | 51.6% | £2,000-£5,500 | low |
| Buccal fat removal | 93 | 48.4% | £1,500-£3,500 | medium |
| Anabolic steroids & TRT | 1,003 | 41.6% | £40-£150/mo | high |
| Rhinoplasty / nose surgery | 57 | 40.4% | £4,000-£9,000 | medium |
| Facial implants (jaw, cheek, chin) | 248 | 39.5% | £3,500-£8,000 | high |
| Dermal fillers | 236 | 36.0% | £150-£600/area | medium |
| Sleep & circadian | 722 | 34.3% | £0 | low |
| Minoxidil | 226 | 33.6% | £10-£25/mo | low |
| Growth hormone & bone-growth stacks | 1,125 | 33.4% | £100-£400/mo | high |
The other 22 interventions are in the report, including the free ones â hydration, supplements, acne â and the ones nobody should touch.
Everything in this table costs four figures or an ongoing monthly bill, and fewer than 45% of the claims about it reach tier 3. Some of it works for the right person. The point is the ratio.

Tier 0 = folklore, 1 = one anecdote, 2 = mechanism story with no result, 3 = consistent human reports, 4 = controlled evidence, 5 = review or trial. Tiers are model-labelled against that rubric â the report says so on its own method page, because a number you can't check is worse than no number. The claim being sold here is a count of what people reported and how strongly, not a clinical finding, and the report says exactly where that stops being useful.
